Shirley Cook, 68, of Lebanon died March 2, 2018 in Lebanon.
She was born April 5, 1949 in Wright County to Floyd Ellis and Laura Mae Wynn Todd.
On Nov. 3, 1967 she was united in marriage to Dale Cook.
She was employed at Mercy Hospital in Lebanon for many years.
She was preceded in death by her father; a brother, Jim Todd and by a great-grandson,Uriah Dale Cunningham.
She is survived by her husband, Dale Cook; one son, Roger Cook and his wife Nancy of Lebanon; one daughter, Toyea Youngblood and her husband Jack of Reeds Spring; her mother Laura Todd of Nixa; three grandchildren, Chelsey Cunningham and her husband Seth, Colton Youngblood and Corey Cook; one great-granddaughter, Emery Sue Cunningham; her sister, Carolyn Cowsert of Nixa; her brother Gregory Allan Todd of Nixa; several nieces and nephews; there relatives and friends.
Funeral services for Shirley Cook will be held at 11 a.m. Tuesday, March 6, 2018 in Shadel’s Colonial Chapel with David Brawley officiating. Burial will be at a later date. Visitation will be at 10 a.m. March 6 until service time at 11 a.m., also at Shadel’s Colonial Chapel. Service is under the direction of Shadel’s Colonial Chapel,
